What are the top five things to consider when creating a safe and effective exercise program?

Exercise.
I go to the gym almost everyday, alternating days between cardio and heavy weights.
I heard that too much cardio can make you lose muscle mass. I do not want this to happen, so do I take between whey protein on a cardio to stop losing muscle mass? Will this actually help?
I have a fairly muscular chest and can bench press my own weight, though I’d like to improve.
My forearms are incredibly weak, although my biceps are quite developed and strong. What might you be able to recommend in order to improve the size and power of my forearms?
I weigh 136 lbs. and I have a BMI of approximately 20.
I have lost as much weight as I think I need from lifestyle changes (water, less junk, etc) over the last 1.5 years.
I need to move to cardio excercise… I think on a daily basis.
Honestly my body has been through a lot and I have only 1 kidney as of 3 years ago.
What instrument should I get to monitor my progress from this starting point?
Any other advice?

i am 13 y/o and i am no doubt… over weight… i need an exercise that is simple and shows full results in 5-8 months … i have alot of time cause i am home schooled… i need an exercise program that does not need any exercise machines cause its too expensive…and i cant go to the gym cause i am too young to go to the gym here…i am from the Philippines and all i see on TV are those weight loss pills and belts that vibrate and i am not into watching shopping channels…i need an exercise program complete with a diet plan… and my budget is 5000 pesos or 100 dollars please help me.. i just need your suggestions ^_^
OK extra edit… i am watching what i eat.. I’ve been watching what i eat… no candies an chocolates…although i eat just a little chocolate once a week…i dont eat rice or bread and i hate pies and cakes…i cant walk outside or inside cause inside… too much stuff… and out side its rainy and its not safe in these streets…cause of countless robberie reports and allot of dogs…and my dad wont let me…

I am referring to blue, gorgonzola, cambozola, etc… I like a lot of different kinds of cheeses, but sometimes the “ripe moldy” ones smell too strong, so unless they have some nutritional value that other cheeses don’t, I can’t see eating them.
Raw garlic is what’s really good for you right? Or is it still just as nutritional after you cook it?
Not in a gym but in the house only.
If all fruits of the same species are grown from seeds that are the same size but are different sizes when picked does this mean that no matter what size it is it will still have the same nutritional value as its larger counterpart?
Please note that I don’t mind consuming extra calories. I am more concerned about the different levels of vitamins, minerals and antioxidants in the two different sizes
